ABSTRACT

According to the literature, biobehavioural interventions or combined strategies would reduce the risk of HIV infection by more than 7,000 new cases each day, which would contribute to the battle facing Latin America, where despite the high incidence and prevalence of HIV in the region, access to PrEP (pre-exposure prophylaxis) is very limited. The objective of this research was, consistent with the PRISMA guidelines (Preferred Reporting Items for Systematic Reviews and Meta-Analyses), to conduct a scoping review to assess the evidence of available outcome data on biobehavioural interventions and programs to address gaps in the PrEP continuum of care in Latin America. The search was conducted in the databases EBSCOhost, WoS, Scopus, and ProQuest, and include all studies published from 2010 to 2021. One hundred eighty-six studies were identified and evaluated according to the eligibility criteria in six stages starting with the elimination of duplicates until the selection of studies that met the previously mentioned criteria; at the end five studies were chosen which were submitted to the methodological quality assessment including compliance with ethical guidelines. Results show that biobehavioural interventions promoting PrEP in Latin America are effective in increasing condom use, decreasing HIV risk, and increasing PrEP awareness, uptake, adherence, and retention. The limited number of interventions in the PrEP continuum of care indicates a need for the uptake of dissemination and implementation science (D&I) frameworks. D&I could help facilitate the translation of evidence-based practices, interventions, and policies to address the HIV crisis in Latin America.


Según la literatura, las intervenciones bioconductuales o estrategias combinadas reducirían el riesgo de infección por VIH en más de 7000 nuevos casos cada día, lo que contribuiría a la batalla que enfrenta América Latina, región en la que, a pesar de la alta incidencia y prevalencia del VIH, el acceso a la PrEP (profilaxis preexposición) es muy limitado. De acuerdo con las directrices de PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses), el objetivo de esta investigación fue realizar una revisión de alcance para evaluar la evidencia de los datos de resultados disponibles sobre intervenciones bioconductuales y programas para abordar las brechas en el continuo de atención de PrEP en América Latina. La búsqueda se realizó en las bases de datos EBSCOhost, WoS, Scopus y ProQuest, e incluyó todos los estudios publicados de 2010 a 2021. Se identificaron ciento ochenta y seis estudios, y se evaluaron de acuerdo con los criterios de elegibilidad en seis etapas, iniciando por la eliminación de duplicados hasta la selección de estudios que cumplieran los criterios previamente mencionados; al final se eligieron cinco estudios, los cuales fueron sometidos a la evaluación de calidad metodológica, incluyendo el cumplimiento de los lineamientos éticos. Los resultados muestran que las intervenciones bioconductuales que promueven la PrEP en América Latina son efectivas para aumentar el uso del condón, disminuir el riesgo de VIH y aumentar la conciencia, la aceptación, la adherencia y la retención de la PrEP. El número limitado de intervenciones en el continuo de atención de la PrEP indica la necesidad de adoptar marcos científicos de difusión e implementación (D&I); marcos que podrían ayudar a facilitar la traducción de prácticas, intervenciones y políticas basadas en la evidencia para abordar la crisis del VIH en América Latina.

ABSTRACT

Background: Diabetes, chronic respiratory diseases, heart diseases, and cancers are diseases of public health importance in India. People with pre-existing non communicable diseases (NCDs) are vulnerable to COVID and associated complications. Risk factors for NCDs were found to have increased due to pandemic. Aims and Objectives: The aim of the study was to study the impact of the COVID 19 on chronic NCDs and life style diseases. Materials and Methods: Articles from India published between March 2019 and May 2022 were searched from database such as PubMed, Scopus, Google scholar using MeSH and keywords such as “COVID 19” OR “Coronavirus” OR “SARS-2 CoVirus” AND Lifestyle diseases,” “COVID-19” OR “Coronavirus” OR “SARS-2 CoVirus” AND “Diabetes” OR “HYPERTENSION.” Summary of 18 articles was presented after inclusion and exclusion criterion and removing of duplicates. Results: Risk of COVID 19 in people with pre-existing non-communicable diseases is high, prognosis is poor, and complications are higher. COVID 19 and lockdown measures resulted in restrictions of movements, dietary and sleep pattern changes, psycho social impacts, non-availability or difficulty in procuring medicines, accessibility to health-care facilities, burden on health-care facilities, difficulties in (using or lack of awareness) about tele-consultations facilities, and thus burden of NCDs is expected to rise in future. Conclusions: COVID 19 pandemic had direct and indirect impact on screening, diagnosis and management of patients with NCDs. Burden of NCDs may rise thereby putting extra burden on health-care facilities which warrants preparedness.

OBJECTIVE@#To re-evaluate the systematic review/Meta-analysis of acupuncture and moxibustion for childhood autism (CA), aiming to provide decision-making basis for clinical diagnosis and treatment.@*METHODS@#The systematic review and/or Meta-analysis of acupuncture and moxibustion for CA were searched in PubMed, EMbase, Cochrane Library, SinoMed, CNKI and Wanfang databases. The retrieval time was from the database establishment to May 5th, 2022. PRISMA (preferred reporting items for systematic reviews and Meta-analyses) was used to evaluate the report quality, and AMSTAR 2 (a measurement tool to assess systematic reviews 2) was used to evaluate the methodological quality, bubble map was used to construct the evidence map and GRADE was used to evaluate the quality of evidence.@*RESULTS@#A total of 9 systematic reviews were included. The PRISMA scores ranged from 13 to 26. The report quality was low, and there was a serious lack in the aspects of program and registration, search, other analysis and funding. The main problems in methodology included not making prespecified protocol, incomplete retrieval strategy, not providing a list of excluded literatures, and incomplete explanation on heterogeneity analysis and bias risk. The evidence map showed that 6 conclusions were valid, 2 conclusions were possible valid and 1 conclusion was uncertain valid. The overall quality of evidence was low, and the main factors leading to the downgrade were limitations, followed by inconsistency, imprecision and publication bias.@*CONCLUSION@#Acupuncture and moxibustion has a certain effect for CA, but the quality of reporting, methodology and evidence in included literature need to be improved. It is suggested to perform high-quality and standardized research in the future to provide evidence-based basis.

ABSTRACT

In this study, an overview of systematic reviews/Meta-analysis(SR/MA) of Chinese herbal injections for sepsis was performed to provide references for clinical practice and promote the quality improvement of clinical evidence. Eight Chinese and English databases such as CNKI, Medline, and EMbase were electronically searched for SR/MA of Chinese herbal injections for sepsis from database inception to June 2022. AMSTAR 2, PRISMA 2020, and GRADE system, combined with Recommendations for Clinical Evidence Grading on Traditional Chinese Medicine Based on Evidence Body, were applied to evaluate the methodological quality, reporting quality, and evidence quality of the included articles. Twenty-seven articles of SR/MA were included, containing four Chinese herbal injections(Xuebijing Injection, Shenfu Injection, Shenmai Injection, and Shengmai Injection). AMSTAR 2 checklist showed that the methodological quality of the SR/MA ranged from moderate to very low. Item 2(prior study design) was the critical item with poor scores, and the non-critical items with poor scores were items 3(explain the selection of the study designs), items 10(report on the sources of funding), and items 16(conflicts of interest stated). In terms of PRISMA 2020, items in eight topics with complete reporting of missing>50%, including search strategy, certainty assessment, results of syntheses, certainty of evidence, registration and protocol, support, competing interests, availability of data, code and other materials. The included SR/MA involved 30 outcome indicators. Evidence quality of mortality, APACHE Ⅱ, and safety, the top three outcome indicators, was evaluated, and all of them were graded as the medium level. The lack of random allocation sequence, allocation concealment mechanism, blinding, and trial sample size was the main reason for the reduction of the evidence level. The available evidence shows that Chinese herbal injections can serve as an effective and safe adjunctive treatment for sepsis, which can reduce mortality, inhibit inflammation, improve coagulation function, and regulate immune function, tissue perfusion, and oxygenation in patients with sepsis. However, the quality of SR/MA was suboptimal, and more high-quality SR/MA is needed to provide evidence to support the efficacy and safety of Chinese herbal injections in the treatment of sepsis.

ABSTRACT BACKGROUND: Carpal tunnel syndrome (CTS) is a common condition greatly affects patients' quality of life and ability to work. Systematic reviews provide useful information for treatment and health decisions. OBJECTIVE: This study aimed to assess the methodological quality of previously published systematic reviews on the treatment of CTS. DESIGN AND SETTING: Overview of systematic reviews conducted at the Brazilian public higher education institution, São Paulo, Brazil METHODS: We searched the MEDLINE and Cochrane Library database for systematic reviews investigating the treatment of CTS in adults. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) and measurement tool to assess systematic reviews (AMSTAR) were applied by two independent examiners. RESULTS: Fifty-five studies were included. Considering the stratification within the AMSTAR measurement tool, we found that more than 76% of the analyzed studies were "low" or "very low". PRISMA scores were higher when meta-analysis was present (15.61 versus 10.40; P = 0.008), while AMSTAR scores were higher when studies performed meta-analysis (8.43 versus 5.59; P = 0.009) or when they included randomized controlled trials (7.95 versus 6.06; P = 0.043). The intra-observer correlation demonstrated perfect agreement (> 0.8), a Spearman's correlation coefficient of 0.829, and an ICC of0.857. The inter-observer correlation indicated that AMSTAR was more reliable than PRISMA. CONCLUSION: Overall, systematic reviews of the treatment of CTS are of poor quality. Reviews with better-quality conducted meta-analysis and included randomized controlled trials. AMSTAR is a better tool than PRISMA because it has a better performance and should be recommended in future studies. ABSTRACT

Abstract Albeit its long trajectory on social sciences and gender studies, female empowerment promotion has gained a renewed protagonism because of its inclusion on the Millenium Development Goals and the Sustainable Development Goals. Among prioritized strategies to achieve this empowerment, international agencies and national governments have focused on gene-rating associative processes around productive activities. This study presents a systematic literature review to analyze the impact of community-based productive rural association's membership over different dimensions of female empowerment: resources, agency, and results. Then, a systematic literature review of five quantitative studies and eight qualitative studies is carried on. The analysis is organized according to each dimension of the empowerment process. In quantitative papers, the standardized size effects are estimated; a content analysis is made for qualitative papers. Results show that impacts of belonging to productive associations have been studied mostly from the empowerment approach as agency -decision-making process-, and for this specific dimension, size effects are statistically significant but small (8=0,05; 0,22).


Resumen Pese a estar presente en diversas discusiones de las ciencias sociales y los estudios de género la promoción del empoderamiento femenino ha alcanzado un renovado protagonismo a partir de su inclusión en los Objetivos de Desarrollo del Milenio y de Desarrollo Sostenible. Dentro de las estrategias priorizadas para lograr este empoderamiento las agencias internacionales y gobierno nacionales han apostado por la generación de procesos asociativos en torno a actividades productivas. En este documento se presenta una revisión sistemática de literatura que tiene por objetivo analizar el impacto de pertenecer a asociaciones productivas rurales en las diferentes dimensiones del proceso de empoderamiento, a saber: recursos, agencia y resultados. Con este fin, se utiliza una metodología de revisión sistemática de literatura de cinco artículos cuantitativos y ocho artículos cualitativos. Para el caso de los cuantitativos, se presenta una organización basada en el componente del proceso de empoderamiento evaluado y se estima el tamaño del efecto estandarizado; para el caso de los cualitativos, se realiza un análisis de contenido de los hallazgos siguiendo la misma organización por dimensiones del proceso. Los resultados muestran que el impacto de la pertenencia a cooperativas ha sido estudiado de forma mayoritaria desde el enfoque de empoderamiento como agencia, entendida como toma de decisiones; y que, para este componente en particular, los tamaños de los efectos (d de Cohen) son significativos, aunque pequeños (8=0,05; 0,22).

BACKGROUND@#Acupuncture has been widely used to relieve migraine-related symptoms. However, the findings of previous systematic reviews (SRs) and meta-analyses (MAs) are still not completely consistent. Their quality is also unknown, so a comprehensive study is needed.@*OBJECTIVE@#To evaluate the reporting and methodological quality of these MAs concerning acupuncture for migraine, and summarize evidence about the efficacy and safety of acupuncture for migraine.@*SEARCH STRATEGY@#PubMed, Embase, Cochrane Library, China National Knowledge Infrastructure, Chinese Biomedical Databases, Wanfang Data, and VIP databases were searched from inception to September 2020, with a comprehensive search strategy.@*INCLUSION CRITERIA@#The pairwise MAs of randomized controlled trials (RCTs) concerning migraine treated by acupuncture or acupuncture-based therapies, with a control group that received sham acupuncture, medication, no treatment, or acupuncture at different acupoints were included.@*DATA EXTRACTION AND ANALYSIS@#Two independent investigators screened studies, extracted relevant data, and assessed reporting and methodological quality using Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2009 and A Measurement Tool to Assess Systematic Reviews 2 (AMSTAR 2), then all results were cross-checked. Spearman correlation test was used to evaluate the correlation between reporting and methodological quality scores.@*RESULTS@#A total of 20 MAs were included in this study. The included MAs indicated that acupuncture was efficacious and safe in preventing and treating migraine when compared with control intervention. There was a high correlation between reporting and methodological quality scores (rs = 0.87, P < 0.001). The quality of the included SRs needs to be improved mainly with regard to protocol and prospective registration, using a comprehensive search strategy, summarizing the strength of evidence body for key outcomes, a full list of excluded studies with reasons for exclusion, reporting of RCTs' funding sources, and assessing the potential impact of risk of bias in RCTs on MA results.@*CONCLUSION@#Acupuncture is an effective and safe intervention for preventing and treating migraine, and could be considered as a good option for patients with migraine. However, the reporting and methodological quality of MAs included in this overview is suboptimal. In the future, AMSTAR 2 and PRISMA tools should be followed when making and reporting an SR with MA.

OBJECTIVE@#To overview the methodological quality, report quality and evidence quality of the systematic review (SR) of acupuncture for vascular cognitive impairment ( VCI ).@*METHODS@#The SRs regarding acupuncture for VCI were searched in PubMed, Cochrane Library, EMbase, CNKI, SinoMed, Wanfang and VIP databases. The retrieval period was from the establishment of the database to September 24, 2020. The report quality, methodological quality and evidence quality of the included SRs were evaluated by PRISMA statement, the AMSTAR 2 tool and the GRADE system.@*RESULTS@#A total of 22 SRs were included, including 102 outcome indexes. The methodological quality was generally low, with low scores on items 2, 5, 7, 10, 14, 15 and 16. The report quality was good, with scores ranging from 19 points to 24.5 points. The problems of report quality were mainly reflected in the aspects of structural abstract, program and registration, other analysis and funding sources. The level of outcome indexes of SRs was mostly low or very low, and the main leading factor was limitation, followed by inconsistency and inaccuracy.@*CONCLUSION@#Acupuncture for VCI is supported by low quality evidence of evidence-based medicine, but the methodological quality and evidence body quality of relevant SRs are poor, and the standardization is needed to be improved.

ABSTRACT

Man's attitudes about the environment have generated irreversible damage to the planet, emerging as an alternative to this problem Environmental Education, which aims to reorient social awareness towards a friendly and thoughtful culture. Through environmental education, we seek to make people aware of the problems of the natural and social environment from their school education in childhood to generate values, new attitudes, behaviors, and beliefs aimed at caring for the environment and learning new relationships between people. Likewise, to carry out these environmental education strategies, it is important to know some specific parameters, such as biological diversity and conservation, in addition to the conservation policies carried out by each nation. In this sense, in this work a bibliometric study was carried out based on high-impact scientific production and stipulated by ScienceDirect related to Environmental Education during a period of the last 20 years. The results were grouped into five clusters: "Environmental Education" OR "Education for Sustainable Development" OR "Education for Sustainability" OR "Education for Climate Change" OR "Eco citizenship". The union of all these clusters are connected and intertwined with each other. Them in a dependent way, which is a consequence of the study carried out(AU)


Las actitudes del hombre sobre el medio ambiente han generado daños irreversibles al planeta, surgiendo como alternativa para esta problemática la Educación Ambiental, que tiene como finalidad reorientar la conciencia social hacia una cultura amigable y reflexiva. Mediante la educación ambiental se busca concientizar a las personas sobre los problemas del ambiente natural y social desde su formación escolar en la niñez para generar valores, nuevas actitudes, comportamientos y creencias orientadas al cuidado del medio ambiente y el aprendizaje de nuevas relaciones entre las personas. Asimismo, para lleva a cabo estas estrategias de educación ambiental, es importante conocer algunos parámetros específicos, como la diversidad biológica y conservación, además de las políticas propias de conservación llevada a cabo por cada nación. En ese sentido, en este este trabajo se realizó un estudio bibliométrico basado en la producción científica de alto impacto y estipuladas por ScienceDirect relacionados con la Educación Ambiental durante un periodo de los últimos 20 años. ABSTRACT

Aggression is an evolutionary behavior as it has a role in survival, increasing one's access to food, shelter, status and reproduction. Testosterone and Cortisol are hormones often linked to aggressive behavior. We gathered and organized data from the last five years on the relation among Testosterone, Cortisol and aggression, while assessing the methods used by those scientific papers. A systematic review was made according to PRISMA guidelines. The search for indexed articles was performed in January 2019 using the keywords aggress* AND Testosterone AND Cortisol in three databases: Web of Science, SCOPUS and PsycInfo. The specific role of Testosterone and Cortisol in aggressive behavior is not unanimous. However, most articles found either an increase in Testosterone or a decrease in Cortisol associated with this behavior. There is the need for standardizing methods of triggering and assessing aggressive behavior, taking into account the assessment of social desirability and its impacts.


Agressividade é um comportamento evolutivo, tendo um papel na sobrevivência ao aumentar o acesso à comida, abrigo, status social e reprodução. A testosterona e o cortisol são hormônios frequentemente associados a comportamentos agressivos. Este estudo reúne e organiza dados dos últimos cinco anos sobre a relação entre testosterona, cortisol e agressividade, avaliando também os métodos utilizados pelos artigos. Uma revisão sistemática foi conduzida segundo as diretrizes do PRISMA. Uma pesquisa eletrônica de artigos foi realizada em janeiro de 2019, usando as palavras-chave agress* AND Testosterone AND Cortisol em três bancos de dados: Web of Science, SCOPUS e PsycInfo. O papel desses hormônios no comportamento agressivo não é unânime, porém a maioria dos artigos incluídos encontrou um aumento na testosterona ou uma diminuição no cortisol, associados a esse comportamento. Há a necessidade de padronizar os métodos de induzir e de avaliar agressividade, levando em conta a desejabilidade social e seus impactos.


La agresividad es un comportamiento evolutivo, por desempeñar papel en la supervivencia, aumentando el acceso a comida, refugio, estatus social y reproducción. La testosterona y el cortisol son hormonas frecuentemente relacionadas con el comportamiento agresivo. Este estudio reúne y organiza datos de los últimos cinco años sobre la relación entre testosterona, cortisol y agresividad, junto a los métodos de evaluación utilizados por los artículos científicos. Se realizó una revisión sistemática de acuerdo con las directrices PRISMA. La búsqueda electrónica de artículos indexados fue realizada enero de 2019, utilizándose de las palabras clave aggress* AND Testosterone AND Cortisol en tres bases de datos: Web of Science, SCOPUS y PsycInfo. El papel de esas hormonas en el comportamiento agresivo no es unánime, aunque la mayoría de los artículos incluídos hayan hallado aumento de testosterona o disminución en cortisol, asociados a ese comportamiento. Se necesita la estandarización de los métodos de inducción y evaluación de la agresividad, teniendo en cuenta la deseabilidad social y sus impactos.

OBJECTIVE@#To evaluate the quality of the existing studies and summarize evidence of important outcomes of meta-analyses/systematic reviews (MAs/SRs) of CFS.@*METHODS@#Potentially eligible studies were searched in the following electronic databases from inception to 1 September, 2019: Chinese Biomedical Literature Database (CBM), China Science and Technology Journal Database (VIP), China National Knowledge Infrastructure (CNKI), WanFang Database (WF), Web of Science, Embase, PubMed and Cochrane Library. Grades of Recommendation, Assessment, Development, and Evaluation (GRADE) was used to evaluate the quality of evidence. The methodological quality of the literature was evaluated by A Measure Tool to Assess Systematic Reviews-2 (AMSTAR-2) and the quality of the report was assessed by Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA). The intra-class correlation coefficient was used to assess the consistency of the reviewers, with an overall intraclass correlation coefficient score of 0.967.@*RESULTS@#Ten MAs/SRs were included. The overall conclusions were that acupuncture had good safety and efficacy in the treatment of CFS, but some of these results were contradictory. The GRADE indicated that out of the 17 outcomes, high-quality evidence was provided in 0 (0%), moderate in 3 (17.65%), low in 10 (58.82%), and very low in 4 (23.53%). The results of AMSTAR-2 showed that the methodological quality of all included studies was critically low. The PRISMA statement revealed that 8 articles (80%) were in line with 20 of the 27-item checklist, and 2 articles (20%) matched with 10-19 of the 27 items.@*CONCLUSION@#We found that acupuncture on treating CFS has the advantage for efficacy and safety, but the quality of SRs/MAs of acupuncture for CFS need to be improved.

ABSTRACT

En este trabajo se hace una revisión bibliográfica sobre el desarrollo evolutivo humano y longevidad, desde un enfoque biopsicosocial (Engel, 1977; Gliedt et al., 2017; Lehman et al., 2017). Tras aplicar el método de análisis PRISMA, se obtuvieron diversos resultados relacionados con un desarrollo evolutivo más longevo; así, en el área biológica, 3 factores: los SNPs, los telómeros y la química del estrés; en el área psicológica, 5 factores: la metacognición, la resiliencia, la espiritualidad, las relaciones personales y la depresión; y en el área social, 8 factores: la pseudo-heredabilidad, las relaciones conyugales, la maternidad, el nivel educativo, estilos de vida, dieta y restricción calórica, actividad física y mental y tecnología sanitaria. Ante los datos obtenidos en las tres áreas, de este enfoque biopsicosocial, y el repetido solapamiento entre factores del área psicológica y del área social, se plantea que pudieran considerarse estas dos como una conjunta, proponiéndose un enfoque explicativo con dos áreas: bio-psicosocial que, por factores encontrados en este trabajo, quedarían un 18,7% de biológica y un 81,3% psicosocial. Actualmente, hay suficiente información sobre desarrollo evolutivo humano y longevidad, pero una ausencia de investigaciones que estudien esos factores desde una perspectiva integrada. Mucha de esa información privilegiada se podría aplicar ya, psicológica y socialmente, a la población en general, para una mejora de su salud, en cualquier fase del desarrollo evolutivo humano.


In this work, a literature review on human evolutionary development and longevity is made from a biopsychosocial approach (Engel, 1977; Gliedt et al., 2017; Lehman et al, 2017). After applying the PRISMA analysis method, several results related to a more long-lived evolutionary development were obtained; thus, in the biological area, 3 factors: SNPs, telomeres and stress chemistry; in the psychological area, 5 factors: metacognition, resilience, spirituality, personal relationships and depression; and in the social area, 8 factors: pseudo-heritability, conjugal relations, motherhood, educational level, lifestyles, diet and caloric restriction, physical and mental activity and health technology. Given the data obtained in the three areas, of this biopsychosocial approach, and the repeated overlap between factors of the psychological area and the social area, it is proposed that both could be considered as a joint, proposing an explanatory approach with two areas: bio-psychosocial that, for factors found in this work, would be 18.7% biological and 81.3% psychosocial. Currently, there is enough information on human evolutionary development and longevity, but an absence of research that studies these factors from an integrated perspective. Much of this privileged information could be applied already, psychologically and socially, to the population in general, for an improvement of their health, at any stage of human evolutionary development.

ABSTRACT

Abstract Background and objective: The systematic review of randomized clinical trials is crucial to assess the safety and effectiveness of intermediate procedures. The objective of this article is to present a tutorial for the planning and execution of systematic review and meta-analysis of randomized clinical trial studies. Method: The systematic literature review is the type of research that organizes, criticizes, and integrates available evidence published in the health field. Systematization leads to less bias, however, the quality of systematic reviews may not always be perceived due to the way it is described in the articles. The information disclosed in the articles is not always free of bias. The steps for carrying out a systematic review include design, protocol registration, implementation, mathematical analysis of results, and dissemination. PRISMA statement has improved the quality of systematic review reports by providing a list of items to be described, and this article emphasizes the key steps for performing a systematic review of interventions. Conclusion: The evidence generated through a systematic review can provide the clinician with greater confidence in decision making at the moment of clinical practice and optimize the benefits to his patients, serving as a tool to assist managers in making decisions regarding the implementation of new strategies for the health of the population.


Resumo Justificativa e objetivo: A revisão sistemática de ensaios clínicos randomizados é crucial para avaliar a segurança e a efetividade das intervenções médicas. O objetivo deste artigo é apresentar um tutorial para o planejamento e execução de revisões sistemáticas e metanálises de estudos de ensaios clínicos randomizados. Método: A revisão sistemática da literatura é o tipo de pesquisa que organiza, critica e integra as evidências disponíveis publicadas na áera da saúde. A sistematização leva a menos vieses, entretanto a qualidade das revisões sistemáticas nem sempre pode ser percebida devido à forma como têm sido descritas nos artigos. A informação divulgada nos artigos nem sempre está livre de vieses. Os passos para a revisão sistemática incluem o delineamento, o registro do protocolo, a execução, a análise matemática dos resultados e a divulgação. O PRISMA statement melhorou a qualidade dos relatos das revisões sistemáticas, pois fornece uma lista de itens a serem descritos, e este artigo enfatiza os principais passos para a execução de uma revisão sistemática de intervenção. Conclusão: A evidência gerada por meio de uma revisão sistemática pode propiciar ao médico maior confiança na tomada de decisões no momento da prática clínica, aprimorar os benefícios aos seus pacientes e servir como ferramenta para auxiliar os gestores na tomada de decisões quanto à implantação de novas estratégias em prol da saúde da população.

ABSTRACT

BACKGROUND: The infratemporal fossa (ITF) is an anatomical lateral skull base space composed by the zygoma, temporal, and the greater wing of the sphenoid bone. Due to its difficult approach, surgical intervention at the ITF has remained a heavy burden to surgeons. The aim of this article is to review basic skull base approaches and ITF structures and to avoid severe complications based on the accurate surgical knowledge. METHODS: A search of the recent literature using MEDLINE (PubMed), Embase, Cochrane Library, and other online tools was executed using the following keyword combinations: infratemporal fossa, subtemporal fossa, transzygomatic approach, orbitozygomatic approach, transmaxillary approach, facial translocation approach, midface degloving, zygomatico-transmandibular approach, and lateral skull base. Aside from our Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) trial, there have been very few randomized controlled trials. The search data for this review are summarized based on the authors’ diverse clinical experiences. RESULTS: We divided our results based on representative skull base approaches and the anatomy of the ITF. Basic approaches to the ITF include endoscopic endonasal, transzygomatic, orbitozygomatic, zygomatico-transmandibular, transmaxillary, facial translocation, and the midfacial degloving approach. The borders and inner structures of the ITF (with basic lateral skull base dissection schemes) are summarized, and the modified zygomatico-transmandibular approach (ZTMA) is described in detail. CONCLUSIONS: An anatomical basic knowledge would be required for the appropriate management of the ITF pathology for diverse specialized doctors, including maxillofacial, plastic, and vascular surgeons. The ITF approach, in conjunction with the application of microsurgical techniques and improved perioperative care, has permitted significant advances and successful curative outcomes for patients having malignancy in ITF.

ABSTRACT

OBJECTIVE: To conduct methodology/reporting quality reevaluation for Meta-analysis/systematic evaluation of anti-TNF-α monoclonal antibody in the treatment of ulcerative colitis. METHODS: Retrieved from the Cochrane library, PubMed, Embase, CBM, Wanfang database and CNKI during data base establishment to Nov. 2018, Meta-analysis/systematic evaluations of anti-TNF-αmonoclonal antibody in the treatment of ulcerative colitis were collected. After data extraction of literatures that meet the inclusion criteria, methodological quality and reporting quality of included studies were evaluated by using AMSTAR scale and the PRISMA statement. RESULTS: Fourteen literatures of Meta-analysis/systematic evaluation were included. The average score of AMSTAR methodology quality (full score of 11 points) was 6.89, with medium methodological quality. PRISMA score (full score of 27 points) ranged from 15 to 26.5. CONCLUSIONS: Meta-analysis/systematic evaluations of anti-TNF-α monoclonal antibody for ulcerative colitis have poor methodological and reporting quality.

ABSTRACT

Objective: This study determined the efficacy of calcitonin gene-related peptide (CGRP) antagonists in the treatment of acute migraine.Methods: Seven randomized, controlled trials were included. Outcome measures used were pain freedom and pain relief two hours after treatment.Results: The difference in pain freedom 2 hours post-dose significantly favored gepants 140/150 mg (OR=2.39, 95% CI=1.93-2.96, PConclusion: With regard to pain freedom and pain relief two hours post-dose, CGRP antagonists are more efficacious than placebo in the treatment of acute migraine but there is insufficient evidence to demonstrate superior efficacy of CGRP antagonists over triptans.

ABSTRACT

To evaluate the report quality of intervention-related systematic reviews or Meta-analysis published in China Journal of Chinese Materia Medica, we searched CNKI and China Journal of Chinese Materia Medica webpages to collect intervention-related systematic reviews or Meta-analysis since the first issue of the magazine. A total of 40 systematic reviews or Meta-analysis reports were included, including one network Meta-analysis. According to the PRISMA statement published in 2009, the report quality of the systematic reviews or Meta-analysis was evaluated. According to the results, 3 had the low quality, 30 had the medium quality, and 7 had the high quality. The average score for all of items was 30 points (21-30.5 points for the medium quality). The 17 high-quality (31-40 points) report items were title, rationale, objectives, information sources, study selection, data collection process, data items, risk of bias in individual studies, summary measures, risk of bias across studies, study selection, study characteristics, risk of bias within studies, results of individual studies, synthesis of results, risk of bias across studies and funding; the 4 medium-quality (21-30.5 points) reporting items were eligibility criteria, search, limitations and conclusions; and the 6 low-quality (<=20.5 points) reporting items were structured summary, protocol and registration, synthesis of results, additional analysis (No.16), additional analysis (No.23) and summary of evidence. Through the analysis, it is found that the report quality of intervention-related systematic reviews or Meta-analysis published in China Journal of Chinese Materia Medica is medium, and it is necessary to improve the quality standard of the report.

Objective To assess the effect of PRISMA statement on intervention-related systematic reviews and meta-analyses published in Evidence-based medicine .Methods Intervention-related systematic reviews and meta-analyses published in Evidence-based medicine from 2001 to 2011 were assessed according to the PRISMA scale and analyzed by Meta Analysist software.Results Seventy intervention-related systematic reviews and meta-analyses involving 14-disease spectra were included in this study.PRISMA statement and systematic reviews and meta-analysespublished by au-thors in colleges and universities could improve their academic level (P<0.05), fund support and the number of authors showed no significant effect on their academic level.Conclusion Literature retrieval methods,literature screening methods,bias assessment methods, and other analyzing methods used systematic reviews and meta-analyses published in Evidence-based medicine and their academic level can be improved by PRISMA statement.

Se trata de una revisión del Prólogo de Amado Alonso, al Curso de Lingüística General de F. De Saussure, donde se muestran las dificultades en la constitución de su objeto, la crítica de la Escuela de Praga y la revalorización del habla. A partir de allí, las incidencias en antropología y psicoanálisis, en éste, paso necesario para formular su teoría del sujeto. Especialmente la trama del lenguaje y la división del sujeto.


This is a review of the Prologue of Amado Alonso, General Linguistics Course F. De Saussure, showing the difficulties in the constitution of the object, criticism of the Prague School speech and presentation. From there, the incidences in anthropology and psychoanalysis, in this, a necessary step to formulate his theory of the subject. Especially the plot of the language and the division of the subject.
